Senior Management and Quality and Safeguards requests that individuals in any of the above roles complete the training. 

The purpose of Client Incident Investigation Training is to provide participants with the skills and resources to conduct a thorough and comprehensive incident investigation which will prevent further incidents and injuries occurring to clients. The training will build capacity in the management team and allow CPA to respond proactively and appropriately investigate serious incidents.

Client Incident Investigation Training will be run by an external organisation Bridge the Gap Training and Consultancy and has been customised to CPA policies and procedures.

What content does the program cover?

On successful completion of this course participants will be able to:

  • Explain manager responsibilities around investigating and reporting on client incidents
  • Compare responsibilities of key agencies, including NSW Police Force, FACS, NSW Health, the NSW Ombudsman, and the NDIS Quality and Safeguards Commission.
  • Plan timely and effective responses to client incidents addressing client safety and wellbeing
  • Examine the use of a fishbone analysis cause and effect diagram to plan an investigation
  • Determine actions and events leading up to an incident, during an incident and following an incident
  • Analyse incidents to determine all the contributing root causes and develop effective control measures to prevent any future recurrences
  • Write an incident investigation report compiling findings and recommendations

Does the program have any pre requisites?

Client Incident Training (either online or face to face) is a pre-requisite for Client Incident Investigation Training.

Course completion requires:

Participation and completion of Client Incident Investigation Report during the session.
